Patient ID
Where do I find my Patient ID?
Your Patient ID is usually printed on your lab result slip, often near the bottom. It may also be your surname plus your birthday.

Spaces, hyphens, and special characters may be removed depending on the clinic record. If you are unsure, please ask WELLSERV staff for help.
First time using the app?
- 1Open the WELLSERV Patient App.
- 2Tap First time? Set up PIN.
- 3Enter your Patient ID.
- 4Enter the General Access Code from staff or your lab result slip.
- 5Choose your own 4-digit PIN.
- 6Confirm your PIN.
- 7Check the consent box.
- 8Tap Save PIN.
- 9Go back to Patient Portal and sign in with your Patient ID and PIN.
Already have a PIN?
- 1Open the app.
- 2Enter your Patient ID.
- 3Enter your 4-digit PIN.
- 4Tap Continue.
Keep your PIN private
Use your PIN only inside the WELLSERV Patient App. Do not send your PIN through chat, comments, or text message.
In the app
Depending on your visit, you may be able to view:
Lab results
View released laboratory results.
Previous results
Check available past or other lab records.
Prescriptions
View prescriptions when available from your visit.
Clinic instructions
See follow-ups or instructions when available.
